Crime Blotter: Prescription Forgery Foiled

A suspect is arrested attempting to obtain pain medication with a forged prescription.

The following reports were taken from the Los Angeles Sheriff's Department, Crescenta Valley Station.

Prescription Forgery

On March 18, Tina Bokazadeh, a 28-year-old Van Nuys resident, was arrested on suspicion of prescription forgery after deputies responded to a call at the , 523 Foothill Blvd.

The pharmacist told deputies that Bokazadeh had requested prescriptions for the pain reliever Hydrocodone/APAP two previous times. On March 15, the prescription was phoned in and filled, but the pharmacist noted the name on the suspect’s identification card differed slightly from what had been called in. According to the pharmacist, Bokazadeh explained it was an error.

Then, on March 16, the pharmacist received another telephone order for the same suspect under the same doctor. The pharmacist filled the order but was suspicious. Upon calling the doctor’s office listed on the prescription, that pharmacist was told that the suspect was not authorized to fill prescriptions under the doctor’s name. The office also reportedly told the pharmacist that Bokazadeh had attempted to obtain prescriptions for the drug from other pharmacies as well.

When Bokazadeh then called in third prescription on March 17, the pharmacist telephoned police. Deputies responded for a forged prescription call, but Bokazadeh did not show up to pick up the drugs. On March 18, Bokazadeh came to the pharmacy, was recognized, and deputies alerted.

After comparing prescription receipts and information in the suspect’s checkbook, it was determined that the suspect could be held. According to the report, Bokazadeh told the deputy she was sorry for what she had done but was in a great deal of pain, needed the medication, and was without insurance. Bokazadeh reportedly told the deputy that to get the medication she had to see a doctor, but could not afford the visit cost, therefore, she had to find another way to get the pills she needed. She also admitted that she had contacted the pharmacy three days in a row in an effort to obtain enough of a supply to manage her pain. Bokazadeh also told deputies that another police agency had recently come to her home and arrested her for the same offense.

During the arrest process, Bokazadeh complained of having chest pains. Bokazadeh was transported by ambulance to the Verdugo Hills Hospital, was treated, and booked for prescription forgery.

Window Smash and Grab

At 9:20 p.m. on March 14, a woman returned to her mini-van parked at the 1930 Foothill Blvd., to discover the middle, passenger window broken. Her purse, hidden beneath a beach towel under the rear passenger seat, had been taken. The purse contained her wallet, driver’s license, Social Security card, credit cards, Nordstrom gift card and two checkbooks.

Ransacked Rooms

A male resident of the 5100 block of Redwillow Lane returned home on the afternoon of March 17 to find his front door unlocked and the entire home ransacked. A glass door in the laundry room had been smashed. Reported stolen were an Xbox and PlayStation, each valued at $450, a laptop computer worth $3500 and $2500 cash.
